Physical exams are required for all students in grades Pre-K/Kindergarten, 1, 3, 5, 7, and 11. New Students are required to provide immunizations and physicals in accordance with New York State Health Department standards. The physical exam must be dated within 12 months prior to the start of the school year, or the date of entrance to the school for new entrants. If a copy of the health examination is not on file within 30 days, you will be notified by the health office. Please note that a current physical is also required for those who wish to participate in sports as well as those requesting working papers.
I recommend that your child’s physical examination be made by your family health care provider who knows your child best. During the doctor visit please check with your child’s physician if your child is due for any mandatory grade level immunizations. If your child has a physical scheduled during the school year, please notify the Health Office with the date of the scheduled physical exam. If you have any questions, please call the Health Office.
